Volunteering at QCS Application form
At Quakertown Christian School, our first priority is to make sure that all of our students are safe at ALL times. The safety and security of our children are of the utmost importance to all of us and it is our collective and collaborative responsibility to work together to this end. In order to accomplish this task, we have implemented a Child Abuse Clearance Policy which can be found below. Thank you for your partnership in educating our children!
- Both the Pennsylvania Child Abuse History Clearances (CY113) and the Pennsylvania Criminal Record Checks (SP4-164) are free for volunteers.
- FBI Fingerprint Record Checks- through Dept. Of Human Services (Public Welfare). There is a fee for this service.
These clearances must be done once every five years. Simply submit a copy of the originals and we will make copies of your clearances so we can keep them on file. This must be done before you can volunteer at the school or chaperon a field trip.
If you have not already applied for these clearances OR if the respective PA Child Abuse (Act 151), Criminal Record (Act 34), and Act 24 form, as well as the FBI clearances, are dated more than five years ago, you must follow the directions below. Once you have completed all clearances using the directions below, please bring the originals to the Main Office to be copied and filed. You will retain the originals.
PENNSYLVANIA CHILD ABUSE HISTORY CLEARANCE INSTRUCTIONS (ACT 151)
The Pennsylvania Child Abuse History Clearance application can be submitted online. Once you enter the link below you will be directed to the Child Welfare Portal where you must create an account or log in if you already have an account. Note: you will need an email address to create an account.
Child Abuse History Clearance Online: https://www.compass.state.pa.us/CWIS
If you need to create an account you must make a Keystone ID which will be your username. You will be e-mailed a temporary password. Use this password to log in, and then you will be asked to change your password. Please remember your Keystone ID and password so you can log back into your account. Be sure to check that you want the original mailed to you. Once you go through the application and submit it you should receive an email stating your application was received and is being processed. **BRING ORIGINAL TO MAIN OFFICE TO BE VIEWED AND COPIED.
PA STATE POLICE CRIMINAL RECORD CHECK INSTRUCTIONS (ACT 34)
Applicants can go to the Pennsylvania Access to Criminal History website and apply for their criminal record check online with the Pennsylvania State Police at: https://epatch.pa.gov/home
Please click on submit “New Volunteer Record Check and follow the directions on the web page. Once you submit this clearance, on the last page, click on the Control # in blue letters next to your name to view the results. Then click on “Certificate Form” in blue letters and print that certificate. If your application is under review, please print out the last page or keep for your records the Control # (starts with an ‘R’), your name as typed in the application, and the date of the submission so you can check on the status of your submission at a later time. **BRING A PRINTED COPY OF YOUR CERTIFICATE TO THE MAIN OFFICE TO BE VIEWED AND COPIED.
FBI CRIMINAL BACKGROUND CHECK INSTRUCTIONS
Applicants must be fingerprinted via the FBI IdentoGo services. Please access these services by scheduling an appointment at a location that is convenient to you. Visit uenroll.identogo.com and use Service Code 1KG6TR to create an account and schedule your appointment. Volunteers will need to pay a fee in order to obtain the proper FBI forms to submit to the office.
