At Quakertown Christian School, it is our heart’s desire to make sure that all of our students are safe at ALL times. The safety and security of our children is of the utmost importance to all of us and it is our collective and collaborative responsibility to work together to this end. In order to accomplish this task, we have implemented a Child Abuse Clearance Policy which can be found below. Thank you for your partnership in educating our children!
Both the Pennsylvania Child Abuse History Clearances (CY113) and the Pennsylvania Criminal Record Checks (SP4-164) are free for volunteers.
FBI Fingerprint Record Checks- $28.00 through Dept. Of Human Services (Public Welfare).
If you already have these clearances for a work or volunteer position and received them within one calendar year (any time after January 1, 2015), you do not need to apply for clearances again. Simply submit a copy of the originals and we will make copies of your clearances so we can keep them on file. This must be done before you can volunteer at the school or chaperon a field trip.
If you have not applied for these clearances OR if the respective PA Child Abuse (Act 151), Criminal Record (Act 34), and Act 24 form as well as the FBI clearances are dated before January 1, 2015, you must follow the directions below. Once you have completed all clearances using the directions below, please bring the originals to the Main Office to be copied and filed. You will retain the originals. This should be done NO LATER THAN December 31, 2015.
PENNSYLVANIA CHILD ABUSE HISTORY CLEARANCE INSTRUCTIONS (ACT 151)
Effective December 31, 2014, the Pennsylvania Child Abuse History Clearance application can be submitted online. Once you enter the link below you will be directed to the Child Welfare Portal where you must create an account or log in if you already have an account. Note: you will need an email address to create an account.
Child Abuse History Clearance Online: https://www.compass.state.pa.us/CWIS
If you need to create an account you must make a Keystone ID, which will be your username. You will be e-mailed a temporary password. Use this password to log in, and then you will be asked to change your password. Please remember your Keystone ID and password so you can log back into your account. Be sure to check that you want the original mailed to you. Once you go through the application and submit it you should receive an email stating your application was received and is being processed. **BRING ORIGINAL TO MAIN OFFICE TO BE VIEWED AND COPIED.
PA STATE POLICE CRIMINAL RECORD CHECK INSTRUCTIONS (ACT 34)
Applicants can go to the Pennsylvania Access to Criminal History website and apply for their criminal record check online with the Pennsylvania State Police at: https://epatch.state.pa.us/Home.jsp
Please click on submit “New Record Check (Volunteers Only) and follow the directions on the web page. Once you submit this clearance, on the last page, click on the Control # in blue letters next to your name to view the results. Then click on “Certificate Form” in blue letters and print that certificate. If your application is under review, please print out the last page or keep for your records the Control # (starts with an ‘R’), your name as typed in the application and the date of the submission so you can check on the status of your submission at a later time. **BRING PRINTED COPY OF YOUR CERTIFICATE TO MAIN OFFICE TO BE VIEWED AND COPIED.
FBI CRIMINAL BACKGROUND CHECK INSTRUCTIONS
The Pennsylvania Department of Human Services is utilizing Cogent Systems to process fingerprint-based FBI criminal background checks. The fingerprint-based background check is a multiple step process. You must REGISTER FIRST online and GET FINGERPRINTED at one of the various locations in Bucks or Montgomery County. Locations are listed below and also online under the PRINT SITE LOCATIONS on the left column of the Cogent website. To register online before you go to the fingerprinting location, click on “REGISTER ONLINE” found in the right column on the Cogent website. Fill out the information and pay for the fingerprinting services. For REASON FINGERPRINTED, you can click down to “Employment with a Significant Likelihood of Regular Contact with Children.” You will also need the following information when registering official ID (license, passport)
Once you have registered, print your registration papers for reference. Visit any of the locations above with a photo ID and your registration paperwork to get your fingerprints. We will make copies of the originals and give you the original for your records.
